I think this could be an interesting illustration using a flat/graphic approach. Consider looking at "Guernica" by Picasso, it is a great depiction of violence and chaos by using shapes instead of rendering. With an approach like that, you can exaggerate anatomy and proportions to give a sense of danger and uneasiness.
The child's pants (the one in his arm) are getting lost in a weird way due to the value being really close to the background. Consider lighting them up a bit to more of a mid tone. I think you can really let loose and purposely distort certain elements for a more dramatic effect.
